Semaglutide Weight Loss in Miami: How a Medical GLP-1 Program Works

Search for semaglutide weight loss in Miami and you will find med spas, telehealth apps, clinics and websites that sell GLP-1s with a checkout button. They are not offering the same thing. A weekly injection is the simple part. The results depend on what happens around it: the tests before the first dose, how the dose is increased, how side effects are handled, and what the plan is once you reach your goal.

This is how a medically supervised semaglutide program works, from the first conversation to the maintenance phase, and what to look for wherever you go.

What semaglutide and tirzepatide do

Semaglutide and tirzepatide are FDA-approved medications for chronic weight management. Both are taken as a once-weekly injection under the skin.

Semaglutide acts on the GLP-1 receptor, the same pathway as a hormone your gut releases after a meal. It slows how quickly the stomach empties and signals fullness to the brain, so smaller portions satisfy you and the constant pull toward food quietens. Tirzepatide acts on GLP-1 and a second receptor, GIP, at the same time.

The trials behind them are large. In the STEP 1 trial, adults taking weekly semaglutide lost an average of 14.9% of their body weight over 68 weeks, against 2.4% with placebo (1). In SURMOUNT-1, adults on the highest dose of tirzepatide lost an average of 20.9% over 72 weeks (2). For a closer comparison of the two, see our guide to the difference between tirzepatide and semaglutide.

Who is a candidate

Semaglutide Weight Loss in Miami: How a Medical GLP-1 Program Works — Rewind Anti-Aging of Miami

Rewind does not use a BMI cutoff. Your provider decides whether a GLP-1 suits you from your labs, your health history and what you are trying to achieve.

Semaglutide and tirzepatide are not used if you have a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, if you are pregnant, planning to become pregnant or breastfeeding, or if you have a history of severe pancreatitis. That is one of the reasons the program starts with a real conversation and a blood panel rather than an online form.

How a semaglutide program works at Rewind

1. A free first conversation. Thirty minutes to talk through your goals and what you have tried before, by video or in our Wynwood office.

2. Consultation and labs. If you move forward, your 60-minute consultation with a provider is included when you run your labs. The blood panel covers metabolic markers, hormones including thyroid, testosterone and insulin, liver function and lipids. It sets your baseline and shows anything that should change the plan.

3. Treatment plan and first dose. Your provider reviews the results, confirms which medication suits you and sets your starting dose. Most patients begin within a couple of days to a week of the first visit. Our team shows you how to give the injection, how to store the medication and what to expect in the first week.

4. Monthly follow-ups. Each month we track your weight, review side effects, move the dose up on schedule and adjust your nutrition and training guidance. Follow-up visits can be done by telehealth.

5. Maintenance. As you approach your goal weight, the plan shifts to keeping the result: gradual dose reduction, continued monitoring and the habits that hold the weight off.

Side effects, and why the dose starts low

The most common side effects are nausea, diarrhea, constipation and reduced appetite. They tend to appear when the dose goes up and settle within a week or two as your body adjusts. This is why every program starts at the lowest dose and increases it in steps rather than jumping to the dose that produces the biggest trial results.

A clinic that follows you monthly can slow the schedule when side effects are strong and speed it up when they are not. Our guide to semaglutide side effects covers each one in more detail.

Protecting muscle while you lose weight

Some of the weight lost on a GLP-1 can come from muscle, not only fat. The scale cannot tell you which. A body composition scan can, because it measures lean mass and fat separately.

Rewind clients have body composition tracked alongside their weight, so a drop in lean mass shows up early and the plan can respond with more protein and resistance training. We explain the approach in GLP-1s and muscle loss.

What happens when you stop

GLP-1s work while you take them. In the STEP 1 extension, adults who stopped semaglutide regained about two-thirds of the weight they had lost within a year (3). That does not mean treatment has to last forever. It means stopping should be planned: a gradual step-down, with nutrition and training already in place, rather than a sudden end. Our article on what happens when you stop semaglutide goes through it step by step.

Why not buy semaglutide online?

GLP-1s are now sold online as “research peptides” with no prescription, no labs and no one following you. The FDA has warned companies selling semaglutide and related products labelled “for research use only” or “not for human consumption” directly to consumers with dosing instructions, and has warned about counterfeit semaglutide that can contain the wrong ingredient, too much or too little of it, or none at all (4). Its advice is to buy only from state-licensed pharmacies.

The risk is not only what is in the vial. A research-grade product comes with no assessment of whether it is right for you, no one to adjust the dose when side effects hit, and no labs to show what is happening underneath. Those are the parts of treatment that decide whether the weight comes off safely and stays off. A seller that will ship you a GLP-1 without asking a single question about your health is telling you how the rest of the treatment will go.

Cost and insurance

Rewind does not accept insurance. HSA and FSA cards are accepted. Costs are discussed during your consultation, so you know what the program includes before you start.

When you compare clinics, ask what each price covers: the medication alone, or the labs, visits, dose adjustments and follow-up as well. Our guide to semaglutide cost explains what to ask.
