Brian Burk had spent his career solving structured problems. The Discovery

He did not accept it, mostly because engineers are bad at accepting unexplained system failures.

So he did what he would have done with any other problem: went to the primary research, read it properly, and looked for a mechanism. What he found was that the specific cluster he was experiencing — fatigue, unexplained weight gain, low mood, cognitive slowing — maps closely onto the symptom profile of hormonal deficiency, and that these are not inevitable features of aging. The Gap

Brian wanted other people to have what he had found. So he went looking at the existing landscape of anti-aging and hormone clinics — as a patient would, and then as an operator would.

What he found was disappointing in a specific and consistent way:

  • Cookie-cutter protocols. The same starting dose and the same product mix regardless of what the labs said, because a standardized protocol is cheaper to run.
  • Thin diagnostics. Minimal bloodwork up front, which makes it impossible to know whether a treatment is correcting anything.
  • No real follow-up. Prescriptions issued, then silence. Nobody rechecking, nobody adjusting.
  • Volume over outcomes. Practices structured around throughput, where the patient is a transaction rather than a case.

He had just spent a year and a half figuring out that the details were the entire thing — which markers, at what level, adjusted how, monitored on what schedule. And the industry was largely skipping the details.

In practice that turned into four operating rules, and they have not changed.

Start with the data. Every plan begins with comprehensive bloodwork, not a protocol. You cannot correct what you have not measured, and you cannot tell later whether something worked if you never established a baseline.

Treat the person, not the range. Lab reference ranges describe the population that got tested, not the target for an individual. Measurement before intervention, without exception. Not a symptom questionnaire followed by a standard protocol. A baseline panel, read against optimal ranges as well as lab ranges, before anything is prescribed. If you cannot measure the starting point, you cannot evaluate the result, and most of the industry is structurally uninterested in evaluating results.

A defined decision point. Every protocol has a scheduled recheck, typically at 8 to 12 weeks, where the question is explicitly “did this work?” rather than “do you want a refill?” Treatments that are not working get changed or stopped.
